Hey, Fusion ISM team! We are so excited to have you on Team Iceland! In order to get you all to and from Iceland with the best experience possible for everyone involved, we need you to fill out the following paperwork items. We ask that you upload any and all documents containing sensitive information directly to our secure online storage via the links provided below. Please do not email sensitive documents. Forms Needed
You can take a picture on your phone, or scan and save as a PDF then upload here. Please ensure that the full name, DOB, expiration date, and passport number are clearly visible in the photo and that you have signed your passport.
Please complete this form as soon as possible so we have ample time to prepare for any dietary restrictions or medical conditions that may require accommodation/additional support. If any pertinent medical information changes between when you fill out the form and the start of the trip, we will ask you to update us with that information.
