Hey, NSCBC team! We are so excited to have you on Team Iceland! In order to get you all to and from Iceland with the best experience possible for everyone involved, we need you to fill out the following paperwork items. We ask that you upload any and all documents containing sensitive information directly to our secure online storage via the links provided below. Please do not email sensitive documents. Onboarding!
These items are due ASAP. Your spot on the trip is not confirmed until we have the deposit, and we will not create your personal fundraising page (if you would like one) until we have received your Confirmation of Intent, Participant Agreement, and Fundraising Agreement.
This is $250 per person. You can pay through this link with a credit card or directly through your bank. We are unable to accept cash, but if you prefer to pay via check please let us know. If you are making a deposit for more than one person, you can simply make as many payments one by one as you have people, or contact Olivia to get a separate link to pay all at once for multiple people.
IMPORTANT: We will use this information to purchase your plane tickets, please double check that your full name and date of birth are entered exactly as they appear on your passport. Please fill out ONE PER PERSON.
Please read this carefully so you are fully aware of what is expected of you while participating in a Compass trip. This can be filled out one per family, as long as everyone has read and understands this form, and everyone signs the form.
This form needs to be printed off (or filled out with a PFD editor), signed and initialed by each parent/guardian, and uploaded. Please save image as PDF, PNG or JPG titled “First Last Parent Agreement” then upload.
This can be a birth certificate, adoption certificate, custody decree, etc. Please save image as PDF, PNG or JPG titled “First Last Child Proof” then upload.
Whether you plan to pay outright or fundraise to cover your expenses, please read and fill it out. This can be completed one per family, signed by the family member(s) taking financial responsibility.

These items (and the above forms) are due no later than May 1st but the sooner you can turn them in, the better.
You can take a picture on your phone, or scan and save as a PDF then upload here. Please ensure that the full name, DOB, expiration date, and passport number are clearly visible in the photo and that you have signed your passport.
Please complete this form ONE PER PERSON as soon as possible so we have ample time to prepare for any dietary restrictions or medical conditions that may require accommodation/additional support. If any pertinent medical information changes between when you fill out the form and the start of the trip, we will ask you to update us with that information.
For the safety of the children we will be serving and for the youth and children on our team, we run a CORI (background check) on all trip participants over the age of 18. Those who will be 18+ at the time of the trip need to download a blank copy of this form from the first button, and upload the completed copy to the second button. The person filling out the form is the “CORI Subject,” and Compass is the “Verifying Employer.” We will use your passport to verify your identity.
