Preparing For Compass

Please save image as PNG or JPG titled “First Last Passport” then upload. (Name, DOB and passport number must be clearly visible in photo!) Due as soon as you receive your passport. If your passport is still being processed, please update us on the status.

This can be a birth certificate, adoption certificate, custody decree, etc. Please save image as PNG or JPG titled “First Last Child Proof” then upload. Not applicable to participants 18+. Due May 16th.

This form needs to be printed off (or filled out with a PFD editor), signed and initialed by each parent/guardian, and uploaded. Not applicable to participants 18+. Due May 16th.

Fill out online form. Please note, some of the content here is intentionally redundant to the La Vida medical form. Please put any pertinent medical information on BOTH forms. Due May 16th.

This form needs to be printed off (or downloaded and filled out on a laptop with a PFD editor), signed, and uploaded. (ONLY for participants who will be 18+ during the program). It’s a background check we run for the safety of those in our group and those we will serve. Due May 16th.

Fill out online form. In the dropdown section at the top of the page, select the “Compass” option. If you will be 18 at the time of the program, you can simply sign the form yourself. If you will be 17 or younger, a parent/guardian will need to fill out the form. Please email kendall.billings@compasspath.org to let me know when you have submitted the form. Due May 16th.
