Hey, LCA Team! We are so excited to bring you to Iceland! In order to get you all to and from Iceland with the best experience possible for everyone involved, we need you to fill out the following paperwork items. We ask that you upload any and all documents containing sensitive information directly to our secure online storage via the links provided below. Please do not email sensitive documents. This is $250 per person. You can pay through this link with a credit card or directly through your bank. We are unable to accept cash, but if you prefer to pay via check please let us know. This deposit counts towards the total amount.
Please read this carefully so you are fully aware of what is expected with regard to the trip.
This form needs to be printed off (or filled out with a PFD editor), signed and initialed by each parent/guardian, and uploaded. Please save image as PDF, PNG or JPG titled “First Last Parent Agreement” then upload.
This can be a birth certificate, adoption certificate, custody decree, etc. Please save image as PDF, PNG or JPG titled “First Last Child Proof of Guard” then upload.
Whether you plan to pay outright or fundraise to cover your expenses, please read this in its entirety and submit it.
You can take a picture on your phone, or scan and save as a PDF then upload here. Please ensure that the full name, DOB, expiration date, and passport number are clearly visible in the photo and that you have signed your passport. If you do not have a passport, please upload any documentation you have showing that you have applied for a passport instead.
