Hey, leaders! We are so excited to have you on Team Iceland! In order to get you all to and from Iceland with the best experience possible for everyone involved, we need you to fill out the following paperwork items. We ask that you upload any and all documents containing sensitive information directly to our secure online storage via the links provided below. Please do not email sensitive documents. These items are due ASAP to verify your spot in the program.
IMPORTANT: We will use this information to purchase your plane tickets, please double check that your full name and date of birth are entered exactly as they appear on your passport.
Please read this carefully so you are fully aware of what is expected of you as a leader on a Compass trip.

These items (and the above forms) are due no later than DATE but the sooner you can turn them in, the better.
You can take a picture on your phone, or scan and save as a PDF then upload here. Please ensure that the full name, DOB, expiration date, and passport number are clearly visible in the photo and that you have signed your passport.
Please complete this form as soon as possible so we have ample time to prepare for any dietary restrictions or medical conditions that may require accommodation/additional support. If any pertinent medical information changes between when you fill out the form and the start of the trip, we will ask you to update us with that information.

For the safety of the children we will be serving and for the youth and children on our team, we run a CORI (background check) on all trip participants over the age of 18. Those who will be 18+ at the time of the trip need to download a blank copy of this form from the first button, and upload the completed copy to the second button. The person filling out the form is the “CORI Subject,” and Compass is the “Verifying Employer.” We will use your passport to verify your identity.
As a Compass leader, you are not required to drive vehicles, but if you are able to it is a big support to the team. Please note, only those who have completed this form and have been approved as drivers will be able to drive Compass vehicles. Read all the conditions carefully and answer honestly before signing the form. You will be asked to submit a copy of your driver’s license and proof of auto insurance upon submitting the form.
