Compass Staff: Instructor

Thank you for completing your staff confirmation form. There are a few more items we will need from you as part of your onboarding process.

As a reminder, your employment with Compass is contingent upon the successful completion of a background check. We also need a copy of your passport and driver’s license. If either of these documents are not up to date, please apply for a renewal immediately, and keep us posted on the anticipated timeline. We also ask that you complete the driver agreement form. To become one of the approved staff members who can drive participants, you need to read and agree to the policies in the Driver Agreement and in addition to your driver’s license, you also need to upload a proof of your auto insurance.

Please download a blank copy of the consent for a CORI check by clicking on the Download button. Then fill it out, and either take a picture or scan the completed form and upload it to the Upload button.
